Assumption of Risk

I, {name}, understand that participation in Brazilian Jiu-Jitsu, yoga, breathwork, and related physical activities at Cherry Blossom Brazilian Jiu-Jitsu & Wellness (“the Academy”) involves inherent risks of injury. These may include, but are not limited to, sprains, strains, bruises, joint or muscle injury, choking, loss of consciousness, and in rare cases, more serious injury.

I acknowledge that I am voluntarily participating in these activities with full awareness of the risks involved and accept full responsibility for my participation.

Medical Clearance

I affirm that I am physically fit and have no medical condition that would prevent safe participation in physical activity. I agree to notify the Academy immediately if my health status changes. I consent to receive basic first aid in the event of injury and understand that emergency medical services may be called at my expense if deemed necessary.

Release of Liability

In consideration of being allowed to participate in classes, open mats, workshops, events, and any other activities associated with the Academy, I hereby release, waive, and discharge Cherry Blossom Brazilian Jiu-Jitsu & Wellness, its owners, instructors, coaches, staff, volunteers, landlords, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may occur while on the premises or participating in any activity, whether caused by negligence or otherwise.

Personal Responsibility

I understand that Brazilian Jiu-Jitsu and related training involve close physical contact. I agree to maintain personal hygiene, respect training partners, follow instructor guidance, and conduct myself in a safe and responsible manner at all times. The Academy reserves the right to refuse participation or terminate membership for unsafe or disrespectful conduct.

I acknowledge that the Academy is not responsible for loss or damage to personal property.

I grant permission for Cherry Blossom Brazilian Jiu-Jitsu & Wellness to use photos or video taken during classes or events for promotional purposes, unless I provide written notice requesting otherwise.
